New Patient Registration
Our practice area primarily covers SK15 postcodes. Please check with reception upon requesting registration to ensure you live within our practice boundary.
To register with the practice you are required to complete both a registration form and a health information questionnaire which gives us helpful information about yourself and your health in general until such time as your medical notes arrive.
Application for registration must be made in person at reception we do not accept emailed registrations. One registration form per person.
New patients are welcome to have a brief health check, please arrange an appointment with one of our healthcare assistants when registering.
You will be allocated a named Doctor once your registration is complete and accepted (this will be your usual Doctor).
